Substance Use Disorder Assessment and Resource Navigation

The Substance Use Disorder Assessment and Resource Navigation is one of 20 Delivery System Reform Incentive Payment (DSRIP) projects Seton is participating in under the Texas 1115 Medicaid Waiver. The DSRIP pool of funds provides payments to hospitals and other providers when they achieve goals intended to improve quality and lower the cost of care. Many of the projects challenge patients and providers to approach care differently.

The Seton DSRIP program on substance use seeks to engage people early on in their road to addiction and substance abuse, providing early intervention that may prevent future health crises. A team of experts on Substance Use Disorders can assess people who may have a substance use problem, coordinate the most appropriate form of care for them, and help them navigate services and resources available to them.

Additional services include:
- Phone follow-up post-discharge to reinforce discharge plans as well as monitor patients' transition out of hospital
- Assistance with overcoming barriers to access needed services
- Ongoing phone follow-up to monitor treatment readiness and provide referrals and support as appropriate

The assessment, care coordination, and navigation services are predominantly for at-risk uninsured and underinsured patients who are receiving care at University Medical Center Brackenridge. The target population is University Medical Center Brackenridge patients (inpatient and emergency department) who have been placed on a detoxification protocol, present with any substance use disorders issues, or are identified by a physician, social service staff member, or project clinician as having potential substance use disorders issues.

To engage patients who may benefit from early intervention and/or education about substance use disorders, and to direct the individuals toward treatment options at the most appropriate levels of care.

The program has administered over 2000 free substance abuse disorder screenings to the target population (University Medical Center Brackenridge patients (inpatient and emergency department) who have been placed on a detoxification protocol, present with any substance use disorders issues, or are identified by a physician, social service staff member, or project clinician as having potential substance use disorders issues).

Demonstration Year 3 (Oct 1, 2013 - Sept 30, 2014): 750 free substance abuse disorder screenings to target population.
Demonstration Year 4 (Oct 1, 2014 - Sept 30, 2015): 850 free substance abuse disorder screenings to target population.
Demonstration Year 5 (Oct 1, 2015 - Sept 30, 2016): 904 free substance abuse disorder screenings to target population.
